The B-Form, also known as the Child Registration Certificate (CRC), is a child’s first official identity document in Pakistan. Issued by NADRA (National Database and Registration Authority), it confirms a child's identity and includes them in the national database.

To help parents, NADRA has now made the B-Form application process easier, especially for newborns. The online registration can now be completed through the Pak ID Mobile App, saving time and avoiding long lines at NADRA offices.

How to apply for B-form online

Parents can now apply online for their child’s B-Form if the child is up to one year old. Here's how:

  • Download and log in to the Pak ID App.
  • Choose “Issuance of Identity Document” and then “Child Registration Certificate (CRC)”.
  • Enter the 13-digit CNIC number.
  • Check the auto-filled data and select your service priority.
  • Upload the child’s photo and your digital signature.
  • Provide the child’s details and upload the required documents.
  • One parent must verify their fingerprints.
  • Review and submit the form.

This step-by-step online process makes child registration in Pakistan faster and simpler.

Eligibility and documents required

Parents need a birth certificate issued by the Union Council and the CNIC or NICOP of at least one parent.

If your child is older than one year, the B-Form application must be done in person at a NADRA office.
